Salary $110,000 - $120,000 per year

Job Details

• Providing and coordinating patient care that is cost-efficient and reimbursable under the patient's insurance plan
• Accepting case assignments based on client needs in specific geographic locations
• Conducting initial assessments, case openings, and documentation on the first day of service
• Traveling to patients' homes and other facilities to deliver direct care
• Completing the Oasis assessment to accurately reflect the clinical and functional status of the patient
• Working with the Supervisor to match HHRG and HIPPS scores to service frequencies and durations
• Participating in weekly PPS meetings to report on patient progress and set discharge dates
  
  

Skills You Need

• Minimum of one year of experience as a Registered Nurse from an accredited institution
• Ability to make home visits in potentially unsafe and unsanitary environments
• Reliable transportation or access to public transportation within Brooklyn, Queens, and Manhattan
• Physical ability to lift up to 50 pounds unassisted and up to 325 pounds with an assistive device
